Frequently Asked Questions

What is Structured Family Caregiving (AFL)?

It's a Connecticut Medicaid program that allows a family member or close friend to be paid for providing daily, hands-on care to a loved one aged 65+ in their shared home — instead of that person moving to an assisted living facility or nursing home.

The person receiving care must be 65 or older and enrolled in Medicaid (Title 19/HUSKY). The caregiver must live in the same household. Contact us and we'll help you find out if you qualify.

Families choose their own caregiver. That person must live in the same household as their loved one, but cannot be a legally responsible relative — such as a spouse or someone holding Power of Attorney.

It's designed to work for both sides: your loved one gets personalized care, financial savings, and independence at home, while you get paid, trained, and supported by our team as their caregiver.

After approval, one of our care coordinators will schedule an in-home assessment to determine the right level of care. From there, caregivers receive training plus ongoing support — including regular check-ins and periodic nursing visits — to make sure everyone has what they need.
